Job summary

Mayo Clinic is seeking a Transplant Coordinator Nurse to oversee care coordination for transplant patients in Jacksonville, Florida. The role involves planning with patients, families, and a multidisciplinary team to ensure seamless care across the continuum.

Qualifications

• Graduate of a baccalaureate nursing program.
• Minimum 1 year of clinical RN experience preferred.
• Must be able to work flexible hours and adapt to unpredictable situations within the patient care setting.
• Demonstrated leadership ability and interpersonal effectiveness required.
• Current RN license by applicable state requirements.
• Maintains Basic Life Support (BLS) competency.
• Coordinators in Transplant must obtain either CCTC or CCTN credential within three years of hire.
• Recertification must be maintained for the duration of the employee’s tenure in the transplant position.

Responsibilities

• Coordinate all aspects of care for the transplant patient population.
• Assess and plan in collaboration with the patient, caregiver, and multidisciplinary healthcare team.
• Provide patient and family education on a consistent basis.
• Provide leadership through activities such as community involvement, committee work, and quality improvement/ research.
• Follow ANA Nursing: Scope and Standards of Practice, UNOS, and FACT guidelines.
