Responsibilities

• Guide patients and families through the transplant journey, from evaluation and workup through surgery preparation and long-term follow-up.
• Conduct detailed patient assessments and monitor for complications before and after transplant.
• Coordinate required testing and clinical logistics to support safe, timely, compliant care.
• Educate patients on medications, self-care, and post-transplant expectations.
• Partner with transplant surgeons, physicians, pharmacists, and social workers to ensure coordinated care and effective transitions.
• Ensure processes align with UNOS and applicable regulatory standards.
• Support quality initiatives and clinical excellence through continuous improvement.
