Job summary

Rocky Mountain Health Care Services is seeking an experienced RN Transitions Nurse to coordinate discharge planning and ensure safe transitions from hospital to home or subacute facilities. You will collaborate with the Transitions Team, IDT, and providers to develop and implement a comprehensive discharge plan and ensure timely referrals.

The role requires an active Colorado RN license, 2+ years of healthcare experience, and strong communication skills to support participants and families

Qualifications

Graduate of an accredited Registered Nurse program and licensed in Colorado.
Two years of healthcare experience in hospital, long-term, or home health settings.
One year of experience with frail or elderly populations preferred.

Responsibilities

Coordinate care from admission to discharge with IDT, SNF, ALF, and hospital discharge planners.
Provide timely updates on hospital status to the provider and healthcare team.
Perform discharge planning meetings and referrals to contracted facilities.
