Job summary

Molina Healthcare seeks an experienced Registered Nurse to provide care transition support, coordinating discharge planning from hospital to home or facility settings. You will follow members through a 30-day program, collaborating with hospital staff, providers and families to minimize Readmissions and ensure quality post-discharge care.

Requires 2+ years health care experience, CTI knowledge, strong communication, and the ability to work independently.

Qualifications

• At least 2 years in health care with 1 year in discharge planning, care management or behavioral health.
• RN license must be active and unrestricted in state of practice.
• Knowledge of or experience with Care Transitions Intervention (CTI).
• Background in discharge planning and/or home health.
• Proactive and detail oriented; able to work in diverse settings.

Responsibilities

• Follows members through a 30-day program from hospital admission through transitions.
• Collaborates with hospital discharge planners, hospitalists, providers, facilities and families.
• Ensures safe transitions with appropriate caregiving and medical oversight.
• Coordinates care with ICT and other service providers for safe transition.
• Conducts face-to-face hospital visits and home visits for high-risk members.
• Educates members on medication management, follow-up care, nutrition and home services.
