Job summary

• Central Registry is required.

Responsibilities

• Provides direct and indirect patient care in the transitional care / skilled nursing setting.
• Communicates with physician/manager/co-workers about changes in patient condition and diagnostic results.
• Performs head-to-toe assessments and reassessments as per policy and unit needs.
• Assesses and manages pain, educates patient and family on pain management.
• Ensures safety and accuracy in all patient care and documentation.
