Job summary

Endeavor Health seeks an RN Transitional Care Navigator in Arlington Heights to lead case management, care coordination, and utilization oversight for a defined patient population. The role focuses on guiding patients through diagnosis, treatment, and post‑acute care within a continuum of care while improving outcomes and reducing costs.

The position is full‑time, 40 hours per week, with a hybrid work model: two days onsite and three days remote optional.

Qualifications

• RN license in Illinois required (IDFPR).
• BLS/CPR certification preferred.
• Experience in case management or population health is valued.

Responsibilities

• Guide high‑risk patients through the health system from diagnosis to follow‑up care to assist navigation of the continuum of care.
• Establish and document individualized care plans using evidence‑based guidelines.
• Coordinate across departments to ensure timely decisions and safe transitions of care.
• Assist with discharge planning, utilization management, and education for patients and families.
