Job summary

You will educate patients, build care plans, and collaborate with care teams and payers to secure approvals and services. The ideal candidate holds an ASN, with BSN preferred, and an Indiana RN license or NLC; 3–5 years in ambulatory or care management

Qualifications

• Requires ASN; BSN preferred.
• Active RN license in Indiana or NLC license to practice.
• 3–5 years of relevant experience, preferably ambulatory care, home health, or care management.
• Phone triage experience preferred.
• BLS certification required; other advanced life support may be required by unit policy.
• Proficiency in Microsoft Office and understanding of medical record requirements.

Responsibilities

• Coordinate care for high-risk patients post-discharge and develop individualized care plans.
• Identify barriers to prevent readmission and ensure smooth transitions of care.
• Conduct thorough phone triage to educate patients and coordinate services.
• Collaborate with third-party payers for coverage and approvals.
• Provide discharge planning documentation and ensure timely follow-up with patients and families.
• Maintain compliance with medical records and regulatory policies.
