Job summary

The Transitional Care Management Coordinator (RN) at Lexington Clinic leads the Transitional Care Management Program for patients transitioning to home care across primary care departments. The position requires KY RN licensure, ADN with BSN preferred, BLS/CPR, and at least two years of relevant clinical experience, with leadership experience considered a plus.

Qualifications

• Requires ADN with KY RN licensure and BLS/CPR certifications.
• Minimum two years clinical experience in primary care, discharge planning, or hospital nursing.
• Leadership/preceptor or supervisory experience preferred.

Responsibilities

• Implement, maintain, and provide clinical leadership for the Transitional Care Management Program.
• Provide direction and oversight of TCM staff, delegating within scope of practice.
• Serve as the clinical resource and escalation point for the team.
