Job summary

Qualifications

Responsibilities

• Coordinates patient care needs by facilitating patient placement in the appropriate nursing unit using pre-established criteria.
• Receives and processes requests for assistance with acute patient transfers and admissions from medical facilities.
• Documents interactions in charting systems and creates reports from patient data.
