Job description

We believe every patient with chronic disease deserves consistent check-ins, follow-up, and support.

Position Overview

The Nurse Chronic Care Coordinator, Remote will perform telephonic encounters with patients on behalf of our partners each month and develop detailed care plans within our care plan templates in the electronic health record. This is initially an Independent 1099 Contractor position with potential to transition to full-time W2 employment with benefits.

Qualifications and Requirements
  • Graduate from an accredited School of Nursing (LPN, LVN, RN, BSN, etc.)
  • Current license to practice as an RN / LVN / LPN with no disciplinary actions
  • Minimum of two years of clinical experience in Med / Surg, Case Management, or home health care
  • Experience with Electronic Medical Records and proficiency with Windows desktop and applications (Microsoft Office 365, Teams, Excel, etc.) in a HIPAA-compliant home environment
  • Strong initiative, judgment, organization, time-management, problem-solving, and decision-making skills
  • High-speed internet and a system running Windows 10 or higher or Mac (no Chromebooks or iPads)
  • Excellent verbal, written, and listening communication skills
Work Details

Part-time, flexible hours, with a minimum of 20 hours per week. Care Coordinators will manage a patient panel of at least 100 patients per month, completing encounters on 90% of assigned patients. Compensation varies based on the number of encounters completed, ranging from $10.00 to $16.00 per encounter, with tier increases requiring two months of consistency.

  • Provide prevention and intervention through motivational coaching
  • Build positive interactions with patients
  • Enhance revenue through billable CCM episodes and increased management visits
  • Develop detailed care plans for doctors and patients
  • Understand healthcare goals related to chronic disease management
  • Attend scheduled meetings (bi-monthly staff meetings, monthly one-on-ones)
