The Nurse Chronic Care Coordinator, Remote will perform telephonic encounters with patients on behalf of our partners each month and develop detailed care plans within our care plan templates in the electronic health record. This is initially an Independent 1099 Contractor position with potential to transition to full-time W2 employment with benefits.
Part-time, flexible hours, with a minimum of 20 hours per week. Care Coordinators will manage a patient panel of at least 100 patients per month, completing encounters on 90% of assigned patients. Compensation varies based on the number of encounters completed, ranging from $10.00 to $16.00 per encounter, with tier increases requiring two months of consistency.
