Job summary

Trilogy Health Services, LLC in La Grange, KY is seeking a Registered Nurse to provide direct, quality clinical care and to lead a team of direct care staff in a long-term care setting.

The RN will monitor residents daily, document care, educate families, and coordinate care from admission to discharge while following facility policies. A valid RN license and CPR certification are required for this role.

Qualifications

• Must hold and maintain a current, valid state RN license.
• Current, valid CPR certification required.

Responsibilities

• Lead a team of direct care providers to ensure correct medication administration, treatments, documentation, family teaching, care planning and patient care conferences.
• Use the nursing process to deliver patient care and ensure continuity from admission through discharge and return to family and community.
• Visit residents daily to assess status and notify the charge nurse, physician, or Director of Nursing of changes in condition.
• Identify resident problems and emergencies and initiate lifesaving measures in the absence of a physician.
• Perform administrative duties such as completing medical forms, reports, evaluations, and charting.
