Job summary

Trilogy’s Greencastle, IN location is seeking a Registered Nurse (RN) to lead a care team and provide direct, high-quality clinical care within a long-term care setting.

You will supervise staff, ensure accurate medication administration and documentation, and coordinate care from admission to discharge while engaging residents and families.

Qualifications

• Must have and maintain a current, valid state RN license.
• Current, valid CPR certification required.

Responsibilities

• Lead a team of direct care providers to ensure medications, treatments, documentation, teaching, and care planning per policies.
• Utilize the nursing process to deliver patient care and ensure continuity from admission through discharge and return to family.
• Visit residents daily to observe status and notify charge nurse, physician, or DON if there is a change in condition.
• Identify resident problems and emergencies and initiate lifesaving measures in the absence of a physician.
• Perform administrative duties such as completing medical forms, reports, evaluations, and charting.
