Job summary

Trilogy Health Services is seeking a Registered Nurse (RN) at Greenleaf Health Campus in Elkhart, IN to provide direct, quality clinical care and to serve as a Team Leader for patient care staff. You’ll collaborate with the care team to deliver safe, compassionate nursing, document carefully, and support families throughout the care journey.

The role emphasizes leading a team, applying the nursing process from admission to discharge, and maintaining licensure and CPR certification.

Qualifications

• Must have and maintain a current, valid state RN license.
• Current, valid CPR certification required.

Responsibilities

• Lead a team of direct care providers to ensure medications, treatments, documentation, and care planning per policies.
• Utilize the nursing process to deliver patient care and ensure continuity from admission through discharge.
• Visit residents daily to assess status and notify charge nurse/physician of changes.
• Identify problems and emergencies and initiate lifesaving measures in absence of a physician.
• Perform administrative duties such as completing forms, reports, and charting.
