Moments Hospice is seeking an experienced RN Clinical Manager/Team Director to lead an interdisciplinary team and ensure seamless, high-quality care for patients and families on the road.

You will oversee care coordination, regulatory compliance, and continuous improvement while collaborating with physicians and the care team. A Bachelor’s degree and 3+ years of healthcare leadership are preferred, with a competitive salary and comprehensive benefits.

Qualifications

• RN license with a minimum of 3 years of healthcare experience; hospice or related setting preferred.
• Bachelor's degree strongly preferred.
• Leadership or management experience is a major plus.

Responsabilités

• Lead and oversee an interdisciplinary team of nurses, aides, social workers, chaplains, and more.
• Ensure adherence to hospice regulations and standards.
• Facilitate communication between medical staff and the care team.
• Lead interdisciplinary meetings, documenting patient needs and care plans.
• Develop and update patient care policies and emergency procedures.
• Manage staff scheduling, performance reviews, and foster a positive work environment.
• Act as the central hub for communication with families, physicians, and other providers.
• Analyze quality metrics and manage a caseload of patients as needed.
• Participate in after-hours and on-call duties to ensure seamless care.
• Maximize resource utilization and promote a collaborative work culture.

• Leadership or management experience a major plus.
• Valid driver's license (required)
