Moments Hospice is seeking an experienced RN Clinical Manager/Team Director to lead an interdisciplinary team in providing compassionate hospice care. You will coordinate care, ensure regulatory compliance, and foster a collaborative environment for patients and families.

Qualifications

• RN license required.
• 3+ years of healthcare experience; hospice preferred.
• Bachelor's degree preferred.
• Leadership or management experience a major plus.
• Valid driver's license required.

Responsibilities

• Lead and oversee an interdisciplinary team of nurses, aides, social workers, chaplains, and more.
• Ensure adherence to hospice regulations and standards.
• Facilitate communication between medical staff and the care team.
• Lead interdisciplinary meetings, documenting patient needs and care plans.
• Develop and update patient care policies and emergency procedures.
• Manage staff scheduling, performance reviews, and foster a positive work environment.
• Act as the central hub for communication with families, physicians, and other providers.
• Analyze quality metrics and manage a caseload of patients as needed.
• Participate in after-hours and on-call duties to ensure seamless care.
