Job summary

Home In The Heart Healthcare Agency is seeking a compassionate RN to conduct supervisory assessments and oversight visits for veterans receiving Homemaker/Home Health Aide services. Service Area includes Hardin County and surrounding areas.

The role requires an active Tennessee RN license, a valid driver's license, and the ability to travel within the service area. PRN/flexible scheduling is offered with mileage reimbursement per policy.

Qualifications

• Current, unrestricted Tennessee RN license.
• Valid driver’s license and reliable transportation.
• Strong clinical assessment and documentation skills.
• Excellent communication and organizational abilities.

Responsibilities

• Conduct initial assessments, supervisory visits, and reassessments.
• Evaluate client health, functional needs, safety, and home environment.
• Develop and update individualized care plans.
• Provide clinical guidance and oversight to caregivers.
• Communicate with clients, families, physicians, case managers, and VA representatives.
• Complete accurate and timely documentation.
• Ensure compliance with agency, VA, and Tennessee regulations.

Home In The Heart Healthcare Agency is seeking a compassionate and experienced Registered Nurse (RN) to conduct supervisory assessments and oversight visits for veterans receiving Homemaker/Home Health Aide services.

Service Area: Hardin County and surrounding areas

Key Responsibilities

• Conduct initial assessments, supervisory visits, and reassessments.
• Evaluate client health, functional needs, safety, and home environment.
• Develop and update individualized care plans.
• Provide clinical guidance and oversight to caregivers.
• Communicate with clients, families, physicians, case managers, and VA representatives.
• Complete accurate and timely documentation.
• Ensure compliance with agency, VA, and Tennessee regulations.

Qualifications

• Ability to work independently and travel within the service area

Preferred

Experience in home health, geriatrics, veterans care, case management, long-term care, or community-based nursing.
