Scion Staffing, Inc. is recruiting for an RN Supervisor to lead clinical care coordination for older adults in Lake Oswego, Oregon. The role supervises a team, collaborates with medical partners, and ensures continuity of care across transitions and settings.

The ideal candidate holds an Oregon RN license, brings geriatric care expertise, and demonstrates strong leadership, communication, and organizational skills. Local travel is required in the Portland metro area.

Qualificações

• Active, unrestricted Registered Nurse (RN) license in Oregon.
• Strong knowledge of geriatric health care and aging-related needs.
• Experience leading and supervising clinical teams.
• Excellent communication, organization, and problem-solving skills.
• Willingness to travel locally across the Portland metropolitan area and use of electronic documentation.

Responsabilidades

• Provide clinical leadership for care coordination for older adults.
• Supervise and mentor a team of clinical staff.
• Coordinate with physicians, hospitals, and home health providers to ensure seamless care.
• Manage care transitions and support plans during hospital discharges.
• Develop partnerships with community providers to improve access to services.
• Contribute to quality improvement initiatives and maintain accurate documentation.
