Soeurs des Saints Noms de Jésus et de Marie seeks an experienced Clinical Care Manager to lead a nursing team providing advocacy, care coordination, and wellness support for Sisters in the Lake Oswego retirement community. You will guide healthcare planning, liaise with medical providers, and supervise CNAs and coordinators to ensure compassionate, comprehensive care.

Qualifications

Graduation from an accredited nursing program; BSN preferred.
RN with 10+ years management and care of older adults’ experience or equivalent.
Unencumbered RN License – State of Oregon

Responsibilities

Assists and advises Sisters needing healthcare assistance; develops health needs-based plans.
Performs care coordination including assessment, planning, implementation, monitoring and adjustment.
Supervises Clinical Care Coordinators and CNAs; coordinates with Physicians and other staff.
Maintains updated communication with Province Director of Healthcare Services and involved staff.
Facilitates and coordinates home care and community-based services; ensures medical appointments and transportation.
Directs Healthcare Proxies/POA for Healthcare; educates and supports DPOA in end-of-life decisions.
