Description
Communicate regularly with residents and their families about Plans of Care, PT, OT, and other treatment protocols. Attend and participate in morning meetings/stand-ups to facilitate communication with the team. Answer residents' questions regarding their care, treatment plans, illness progression, and other issues to ensure they feel safe and secure in our care.
Monitor and adjust residents' statuses based on changing needs and conditions. Organize and prioritize daily work by assessing the needs of new, current, and discharging residents within areas of responsibility. Complete documentation as required. Perform utilization review activities to ensure resident-appropriate, timely, and cost-effective care. Coordinate care with residents, care providers, facility financial services, and third-party payers. Oversee all admissions and discharge activities and coordinate referrals to and from our facility.
Maintain positive, effective, and appropriate relationships with residents, families, staff, and professional colleagues. Ensure accurate charting and complete necessary paperwork in a timely manner. Work independently, exercising sound judgment in interactions with physicians, providers, payers, residents, and their families. Promote cooperation and collaboration among all parties involved. Demonstrate excellent time management skills to develop organized work processes in a high-volume environment with rapidly changing priorities. Possess intermediate computer skills and maintain professional, respectful, honest interactions with residents, families, staff, and partners.
Supervisory Requirements: This position has no supervisory responsibilities.
Qualifications: Bachelor's Degree in Nursing or Social Work preferred. Licensed (LVN, LPN, or RN) in the state of employment. Two (2) years of clinical nursing experience preferred. Knowledge of Medicare, Medicaid, and related programs. PCC knowledge is a plus.
Language Skills: Ability to read and comprehend technical procedures, policy and procedure manuals; effectively present information and respond to questions from managers and employees.
Mathematical Skills: Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations.
Reasoning Ability: Ability to solve practical problems and interpret instructions furnished in various formats.
Certificates, Licenses, and Registrations: Valid LVN/LPN or RN license. Certification as a Case Manager (CCM) is a plus. Must maintain all required continuing education and licensing, and remain in good standing with the Department of Public Health.
Physical Demands: Frequent standing, walking, reaching, pushing/pulling, talking, hearing, tasting, and smelling. Ability to lift up to 50 pounds. Occasional climbing, balancing, stooping, kneeling, crouching, or crawling. Visual acuity for close, distance, color, peripheral, and depth perception.
Work Environment: Usually low to moderate noise level. Reasonable accommodations available for individuals with disabilities. Management may reassign duties as needed.
