White Blossom Care Center in San Jose, CA seeks an RN Supervisor for AM and PM shifts. You will provide clinical leadership to frontline staff, oversee admissions, and ensure daily operations run smoothly in a resident-centered SNF.

Candidates must hold a California RN license and CPR/BLS, with prior charge nurse or supervisory experience in SNF or long-term care. PCC experience is a plus; Title 22 familiarity preferred.

Competenze

• California RN license required.
• Active CPR/BLS certification.
• Supervisory/charge nurse experience in SNF/long-term care preferred.
• Experience with PCC for electronic health records and audits is a plus.
• Familiarity with Title 22 long-term care regulations.

Mansioni

• Coordinate and complete resident admissions and oversee clinical documentation.
• Supervise, mentor, and provide performance feedback to nurses and CNAs.
• Conduct digital clinical audits within PCC to ensure medical record accuracy.
• Maintain shift-wide compliance with Title 22 and federal guidelines.
• Collaborate with physicians and the care team to update resident care plans.
