Ideal candidates hold a CT RN license with SNF experience and demonstrate strong leadership, communication, and collaboration with the health care team to ensure safe, effective care delivery and thorough documentation.

Formación

• Valid CT RN license in good standing.
• Previous SNF experience strongly preferred.
• Ability to lead a fast-paced healthcare team.
• Strong communication and leadership skills.

Responsabilidades

• Lead a team of nurses and provide guidance for high-quality care.
• Develop and implement individualized resident care plans.
• Collaborate with the healthcare team for seamless care delivery.
• Provide direct patient care and supervise nurses and caregivers as needed.
• Monitor and maintain accurate records and documentation.
• Participate in quality improvement initiatives and inventory management.
• Communicate effectively with residents, families, and other healthcare professionals.
