You will oversee daily operations, mentor staff, and coordinate care plans across interdisciplinary partners to support positive patient outcomes. The role emphasizes leadership, adherence to policies, and ongoing quality improvement.

Responsibilities

• Supervise and coordinate the activities of nursing staff to ensure that patient care is delivered in accordance with our center's policies and procedures
• Develop and implement individualized care plans for patients, ensuring that their unique needs are met and exceeded
• Conduct regular rounds to assure quality care, identify areas for improvement, and implement corrective actions
• Provide guidance, support, and mentorship to nursing staff to enhance their skills and knowledge
• Collaborate with other departments, including therapy, social work, and activities, to ensure comprehensive care
• Participate in peer review and quality assurance initiatives to continuously improve patient outcomes
• Develop and maintain relationships with community hospitals, such as Albert Einstein Medical Center and Temple University Hospital, to ensure seamless admissions and transitions
