Job summary

Swan Lake Nursing & Rehabilitation in Patchogue, New York, is seeking a dedicated RN Supervisor to lead our nursing team. You will oversee LPNs and CNAs, maintain documentation of resident care, and help establish compassionate care plans.

You will support residents and families, ensure regulatory compliance, and uphold standards of care across units. The role offers a 3pm–11pm schedule with full-time/part-time flexibility and competitive hourly compensation.

Qualifications

• Long-term Care experience preferred.
• Valid RN license required.
• 2-4 years of management experience.
• Knowledge of City, State, and Federal regulations pertaining to licensed skilled nursing facilities.

Responsibilities

• Establish a compassionate environment by providing support to Residents and their families.
• Maintain documentation of Resident care services by auditing Resident and department records.
• Create appropriate care plans for residents.
• Handle all supervisory duties for assigned units, including overseeing LPNs and CNAs that report to you.
• Help establish and implement employment policies and procedures.
• Maintain the standards of care for the units.
• Administer medication as needed, review Resident records, and monitor the quality of care.
• Address questions or complaints brought forward by Residents or their families.
