Job summary

The Paramount at Somers Rehabilitation and Nursing Center in Somers, NY is seeking an experienced RN Supervisor to lead nursing operations on your shift and drive clinical excellence. You will mentor staff, ensure compliant care, coordinate admissions, and collaborate with physicians to promote positive outcomes for residents.

Qualifications

• Current and active Registered Nurse (RN) license in NY.
• Supervisory or leadership experience in Skilled Nursing Facility, Rehabilitation Center, or Long-Term Care preferred.
• Strong clinical judgment, assessment and critical thinking skills.
• Knowledge of long-term care regulations and survey processes.

Responsibilities

• Supervise RNs, LPNs, and CNAs during assigned shifts.
• Ensure residents receive safe, compassionate, and individualized nursing care.
• Provide clinical guidance and leadership to the nursing team.
• Monitor resident conditions and respond to changes in health status.
• Coordinate admissions, discharges, and transfers.
• Review and maintain accurate clinical documentation.
• Ensure compliance with CMS, NYS DOH, and facility policies.
• Collaborate with physicians and interdisciplinary team to promote positive resident outcomes.
• Assist with staffing coordination and daily nursing operations.
• Support quality assurance, infection prevention, and risk management initiatives.
• Maintain survey readiness and assist during state and federal surveys.
• Foster a positive, respectful, and collaborative work environment.

The Paramount at Somers Rehabilitation and Nursing Center is seeking an experienced and motivated RN Supervisor to join our nursing leadership team. As an RN Supervisor, you will oversee nursing operations during your assigned shift, provide clinical leadership, mentor nursing staff, and ensure the highest standards of care for our residents.
