Job summary

Apex Rehabilitation & Care Center is seeking a passionate Registered Nurse (RN) Supervisor to join our team. You will guide certified nursing assistants and LPNs, review care plans, and participate in discharge planning for our long-term care facility.

We offer 12-hour shifts, weekend supervision, generous PTO, health and dental benefits, a 401(k), and ongoing training. NY RN licensure is required; BSN preferred and LTC experience strongly preferred.

Qualifications

• Current New York State RN license.
• BSN preferred; associates acceptable with LTC experience.
• Strong communication and leadership skills; LTC experience required.

Responsibilities

• Supervise CNAs and LPNs; coordinate patient care plans.

Registered Nurse Job description:

Part-time/Full time 7am-7pm and 3pm-11pm, 11pm-7am

Apex Rehabilitation & Care Center, is looking for a passionate and hard-working Registered Nurse (RN) Supervisor to join our team! We offer 12 hours shifts for supervision on weekends for LTC facility. Must have LTC experience.

Responsibilities for a Registered Nurse Supervisor:

As an Registered Nurse, you will guide and direct the activities of certified nursing assistants and LPN's, review and modify care plans in collaboration with patients, families, and the interdisciplinary team, attend care planning and discharge planning reviews, and provide patient/family education.

You will also be responsible for administering medications and treatments, performing special tests, documenting treatment, and responding to patient care emergencies.

• Perform required skills (assessment, intervention) according to guidelines, forms, policies and procedures, follow DOH guidelines.
• Develop care plan that demonstrates knowledge of criteria to ascertain residents specific needs based on health care problem(s)
• Carry out care plan that demonstrates familiarity with criteria to establish resident priorities specific to anticipated outcomes
• Implement physicians’ orders; administer medications, start IVs, perform treatments, procedures and special tests and document treatment as required by company policy and local/state/federal rules and regulations

Qualifications of a Registered Nurse (RN)

• Current New York State Registered Nurse (RN) license
• BSN preferred (associates is excepted with experience in LTC).
• Must have effective communication skills, both verbal and written and long term care experience.
• Must be creative and have good observations and analytical skills with leadership skills.
