Job summary

Community Hospital of San Bernardino seeks an RN for sub-acute care to provide total patient care and support patients’ recovery goals on a dedicated unit. You will apply the nursing process to deliver comprehensive care and coordinate services with the healthcare team to optimize outcomes.

Success requires active participation in fulfilling the hospital’s mission and core values, identifying nursing service problems, maintaining patient care standards, and adhering to policies and regulatory

Qualifications

  • Minimum of 6 months RN experience in acute or long term care.
  • Current and Active CA RN License.
  • Current BLS certification.

Responsibilities

  • Comprehensive Patient Assessment: Conduct timely and thorough patient assessments, including psychosocial and cultural needs, to develop individualized, age-specific care plans.
  • Direct Patient Care & Medication Administration: Provide basic patient care, execute physician's orders, and safely administer medications, including narcotics and blood products, with awareness of drug interactions.
  • Emergency Management & Problem Solving: Lead during emergencies, proactively address issues with the healthcare team, and intervene for high-risk patient problems.
  • Accurate Documentation & Communication: Document all nursing interventions and patient responses effectively, and communicate critical information to physicians and colleagues promptly.
  • Patient & Family Education: Educate patients and families on illness management and post-discharge care, including assessing and addressing complex learning needs.
  • Outcome Evaluation & Quality Improvement: Evaluate intervention effectiveness, recommend treatment improvements, and assess patient/family understanding of care.

Job Summary and Responsibilities

As our RN Sub Acute Care, you will help patients on a sub-acute care unit receive total patient care so they can achieve optimal health outcomes and recovery.


Everyday you will utilize the nursing process to provide comprehensive care and coordinate services from the healthcare team.


You will be expected to identify and correct nursing service problems, maintain patient care standards, and adhere to hospital policies and regulatory guidelines.
