Rockport Healthcare Services in Oakland, CA seeks an experienced RN to provide direct nursing care in a long-term care setting under the supervision of the Director of Nursing Services.

The role requires admitting, assessing, planning, implementing, and evaluating resident care; supervising CNAs; administering medications; ensuring documentation accuracy; and participating in meetings and quality initiatives. A CA RN license and CPR certification are required.

Qualifications

• Graduate of an accredited RN program.
• Current nursing license in good standing with the state board.
• Demonstrated leadership, communication, and organizational abilities.
• Strong critical thinking to assess, plan, implement, and evaluate care.
• Solid knowledge of nursing principles and standards of care.
• Fluent English communication (reading, writing, speaking).
• Valid CPR certification.

Responsabilités

• Provide nursing care per physician orders within scope of practice and standards.
• Administer professional services to support residents’ physical, mental, and emotional well-being.
• Record and document care accurately and timely in the medical record.
• Initiate, review, revise, and update Plans of Care as needed; monitor patient progress.
• Assist in rounds with attending physician;Transcribe orders accurately.
• Prepare and administer medications as ordered; monitor responses.
• Supervise CNAs and assist with nursing assignments and performance.
• Attend care conferences, staff meetings, and QA activities.
