Job summary

Avamere Rehab of Oregon City is seeking a Registered Nurse (RN) for a PRN role with a Wednesday day shift from 6am to 6pm at our Oregon City, OR location. The RN will provide comprehensive patient care, assess residents, administer medications, monitor health, and coordinate care with the healthcare team.

The ideal candidate holds a nursing degree, active OR RN license, CPR/BLS, and has experience in long-term care or geriatric rehab.

Qualifications

• Must possess a nursing degree from an accredited college or university.
• Must have an active RN license in Oregon; BSN preferred.
• CPR/BLS certification required during employment.
• Experience in long-term care or geriatrics is preferred.
• Experience with electronic medical records documentation.

Responsibilities

• Assess residents and develop individualized care plans with the interdisciplinary team.
• Administer medications and monitor responses for safety and efficacy.
• Monitor vital signs and document changes in condition promptly.
• Coordinate care with physicians, therapists and other professionals.
• Supervise LPNs and CNAs; provide orientation and training for staff.
• Maintain accurate documentation and communicate with families.
• Participate in quality improvement and stay current with best practices.
