Signature HealthCARE of Spencer County, a 120-bed skilled nursing facility in Crestwood, KY, is seeking an experienced RN to join our team delivering high-quality resident care, coordinating with an interdisciplinary team, and supervising nursing assistants.

The role emphasizes medication administration, ADLs, documentation, and regulatory compliance; CPR/BLS preferred, 1 year clinical nursing experience, and knowledge of EHR systems.

Qualifications

• Current, active RN license is required.
• CPR/BLS certification preferred.
• Minimum one year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory/charge nurse role preferred.
• Strong clinical observation, medication administration, and documentation skills.
• Knowledge of Federal and State long-term care regulations.
• Familiarity with electronic health records (EHR) systems.
• Proficiency with Microsoft Office applications.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with activities of daily living (ADLs) ensuring comfort, dignity, and safety.
• Supervise nursing assistants and medication aides during shifts.
• Monitor residents’ status and notify physicians of changes.
• Develop and review individualized care plans with the interdisciplinary team.
• Complete accurate clinical documentation per policy.
• Coordinate admissions, transfers, and discharges.
• Participate in quality assurance activities and regulatory surveys.
