Job summary

Signature HealthCARE in Lexington is seeking both experienced RNs and new grads for full-time and part-time opportunities. You will provide direct nursing care to residents, administer medications, perform skilled nursing procedures, and support ADLs in a long-term care setting.

You will supervise nursing assistants during shifts, maintain accurate documentation, coordinate admissions and discharges, and collaborate with an interdisciplinary team to deliver person-centered care while complying

Qualifications

Active RN license in state of practice.
Experience in long-term care or skilled nursing preferred.
Supervisory or charge nurse experience preferred.
Strong observation, medication administration, and documentation skills.
Knowledge of federal and state long-term care regulations.

Responsibilities

Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
Assist residents with ADLs and ensure comfort, dignity, and safety.
Supervise nursing assistants and medication aides during shifts.
Monitor residents and report changes to physicians and families.
Develop and review individualized care plans with the interdisciplinary team.
Document accurately in EHRs and facility systems.
Coordinate admissions, transfers, and discharges.
Participate in QA activities and regulatory surveys.
