Qualifications

• Active RN license in Kentucky.
• CPR/BLS certification preferred (AHA/Red Cross).
• Minimum one year of clinical nursing experience; long-term care preferred.
• Supervisory or charge nurse experience preferred.
• Strong observation, med administration, and documentation skills.
• Knowledge of Federal and State long-term care regulations.
• Effective verbal and written communication.
• Proficiency with EHR systems and MS Office.
• Ability to work independently and prioritize tasks.

Responsibilities

• Provide direct nursing care and med administration.
• Assist residents with ADLs and ensure dignity and safety.
• Supervise nursing assistants and medication aides.
• Monitor residents and report changes to physicians.
• Develop and review individualized care plans.
• Complete accurate clinical documentation.
• Coordinate admissions, transfers, discharges.
• Participate in QA activities and staff education.
• Perform other duties as assigned.
