Job summary

Baldwin-Gardens-Nursing-Center in Temple City, CA is seeking a skilled Registered Nurse (RN) to provide direct patient care in a busy Skilled Nursing Facility. You will assess, monitor, and document residents’ conditions while collaborating with the broader health care team to meet emotional, physical, and cognitive needs.

You will develop individualized care plans, administer medications, coordinate with physicians and therapists, and educate residents and families.

Qualifications

• Valid California RN license required.
• Graduate of accredited nursing program.
• Minimum of 1 year experience in a skilled nursing facility, acute care, or inpatient rehab/behavioral health.
• Proficient with clinical software such as American Health Tech (AHT).
• Strong clinical and interpersonal communication skills.

Responsibilities

• Provide direct nursing care to residents, including administering medications and monitoring vital signs.
• Develop and implement individualized care plans based on residents’ needs.
• Coordinate care with physicians, therapists, and other healthcare professionals.
• Ensure accurate, complete, and up-to-date documentation and charting.
• Assist with activities of daily living (ADLs) as needed and educate residents/families.
• Respond to emergencies and provide timely first aid.
• Adhere to all policies, procedures, and regulations governing nursing care in the facility.
