Job summary

Baldwin Gardens Nursing Center in California is seeking a skilled Registered Nurse (RN) to join our team at a busy Skilled Nursing Facility. You will provide direct nursing care, administer medications, monitor residents, and document changes in their condition according to professional standards and facility policies.

The ideal candidate holds a valid California RN license, has at least one year of SNF or inpatient experience, strong clinical skills, and proficiency with AHT or similar

Qualifications

• Valid and current California RN license.
• Graduate of an accredited nursing program.
• Minimum 1 year experience in skilled nursing, acute care, or inpatient rehab/behavioral health.
• Proficient with clinical software such as AHT.

Responsibilities

• Provide direct nursing care to residents, including medications and treatments, monitoring vital signs, and documenting condition changes.
• Develop and implement individualized care plans based on residents' needs and goals.
• Coordinate care with physicians, therapists, and other healthcare professionals.
• Ensure documentation and charting are accurate and up-to-date.

Job description

We are seeking a highly skilled and compassionate Registered Nurse (RN) to join our team at a Skilled Nursing Facility. As an RN, you will be responsible for ensuring that residents' needs are met in accordance with professional standards of practice through physician orders, facility policies and procedures, and federal, state, and local guidelines. You will observe, evaluate and gather data, confer with and be directed by the Director of Nursing to meet emotional, physical, and cognitive needs of the patients.

Responsibilities:

• Provide direct nursing care to residents, including administering medications and treatments, monitoring vital signs, and assessing and documenting changes in their condition
• Develop and implement individualized care plans based on residents' needs and goals
• Coordinate care with other members of the healthcare team, including physicians, therapists, and other healthcare professionals
• Ensure that all documentation and charting is accurate, complete, and up-to-date
• Assist with activities of daily living (ADLs) as needed
• Educate residents and families about their healthcare needs and treatments
• Respond to emergencies and provide first aid as needed
• Adhere to all policies, procedures, and regulations governing nursing care in the facility

Qualifications:

• Valid and current RN license in the state of California
• Graduate of an accredited nursing program
• Minimum of 1 year experience in a skilled nursing facility, acute care setting, or in-patient rehabilitation or in-patient behavioral health setting
• Strong clinical skills, including assessment, planning, and implementation of nursing care
• Excellent communication and interpersonal skills
• Proficient in using our clinical software American Health Tech (AHT) or similar.
