Job summary

Avamere Skilled Advisors, LLC d/b/a Avamere Living is seeking a Registered Nurse (RN) for the SNF in King City. The role is full-time with a day shift from 6 am to 2 pm at Avamere Rehab of King City, Tigard, OR.

Responsibilities include resident assessment, medication administration, care planning, and interprofessional coordination. The RN will supervise LPNs and CNAs, maintain documentation, monitor vital signs, and contribute to quality care in a skilled nursing setting.

Qualifications

• Must possess a nursing degree from an accredited college or university.
• Must possess a current, active RN license in the state. BSN preferred.
• Must maintain active CPR/BLS during employment.
• Experience in long-term care, geriatric rehab, sub-acute or critical care setting preferred.

Responsibilities

• Assess residents and develop individualized care plans with the interdisciplinary team.
• Administer medications and monitor responses for safety and effectiveness.
• Monitor vital signs and document changes in condition.
• Supervise LPNs and CNAs and provide staff orientation and training.
• Maintain accurate, complete documentation and coordinate care with physicians and other professionals.
