This position covers patients in Springfield, MO.

As a Home Health Registered Nurse, You Will

• Provide admission, case management, and follow-up skilled nursing visits for home health patients.
• Administer ongoing care and case management for each patient, providing necessary follow-up as directed by the Clinical Manager.
• Confer with physicians to develop initial treatment plans based on orders and assessments. Provide hands-on care, management, and evaluation of the care plan, along with patient teaching, under supervision. Revise plans as needed in consultation with physicians.
• Coordinate appropriate care with various healthcare personnel (e.g., Physical Therapists, Occupational Therapists, Home Health Aides, external providers).
• Report patient care, condition, and progress to physicians and the Clinical Manager regularly.
• Implement patient care plans collaboratively with patients and families to achieve optimal outcomes.
• Supervise and coordinate the work of Home Health Aides, ensuring documentation is completed accurately.
• Discharge patients after consultation, preparing necessary documentation.
• Maintain comprehensive medical documentation, including case conferences, medication changes, re-certifications, and updates.

Required Experience/Skills

• Diploma, Associate, or Bachelor Degree in Nursing
• Minimum of one year nursing experience preferred; home health experience required
• Proficiency with OASIS
• Current, unrestricted RN license
• Current CPR certification
• Strong organizational and communication skills
• Valid driver’s license, auto insurance, and reliable transportation

Full-time visits in Springfield, MO.

Pay Range

• $45.00 - $63.00 per visit/unit
• $70,500 - $96,900 per year base pay
