Job summary

One Senior Care is seeking a Registered Nurse to provide direct and indirect nursing care to participants, including home visits. You will manage complex clinical situations, develop care plans, educate families, and document outcomes in the EMR.

This role suits a results-driven, organized professional who thrives in a fast-paced healthcare setting and collaborates with interdisciplinary teams to support participant well-being.

Qualifications

• Graduate of an accredited nursing program.
• Current RN license in practicing state.
• Experience in complex nursing care and interdisciplinary collaboration preferred.
• Experience with frail or elderly population is a plus; hands-on training provided.

Responsibilities

• Provide direct and indirect nursing care to participants following the Care Plan.
• Collect and analyze patient data, set priorities, and identify problems needing follow-up.
• Develop, implement, and evaluate plans of care, collaborating with interdisciplinary teams.
• Provide education, guidance, and discharge instructions to participants and families.
• Respond independently to emergencies and implement advanced care as needed.
• Document care, observations, and participant outcomes accurately in the electronic medical record.
• Communicate participant changes and updates clearly to the care team.
• Participate in Quality Improvement initiatives and clinical process enhancements.
• Travel to participant homes or other sites as needed to provide care.
