Job summary

One Senior Care is seeking a Registered Nurse to provide direct and indirect nursing care to participants, navigating complex clinical situations with confidence. This role emphasizes collaboration within a fast-paced team and delivering safe, effective care.

The position includes approximately 20% travel to participant homes, a first shift, and full-time hours with comprehensive benefits. You will work with specialized care teams across Buckeye PACE, LIFE-NWPA, Mountain View PACE, and PACE

Qualifications

Must be a graduate of an accredited nursing program and hold an active RN license in practicing state.
Experience delivering complex nursing care and collaborating with interdisciplinary teams preferred.
Experience with geriatric or frail populations is a plus, with mentoring provided.

Responsibilities

Provide direct and indirect nursing care to participants per Care Plan.
Collect and analyze patient data, set priorities, identify issues needing follow-up.
Develop and evaluate plans of care with interdisciplinary teams.
