Job summary

Fairview Park Hospital is seeking a Transition to Practice Coordinator to ensure RN Residency and related programs run smoothly. You will serve as the primary contact, coordinate onboarding, and partner with clinical education leadership to implement evidence-based content for professional practice.

You will lead onboarding, support retention, verify participants meet requirements, and meet stakeholders regularly.

Qualifications

• Bachelor’s degree in Nursing Required
• Master’s degree in Nursing or Education; if the graduate degree in not in nursing, the undergraduate degree must be in nursing. Preferred
• 5+ years of experience in an acute care facility Preferred
• 3+ years as a clinical educator or preceptor Preferred

Responsibilities

• Leads coordination, managing, facilitating, conducting, and evaluating onboarding and orientation programs for the new Registered Nurse within assign site(s).
• Provides and assists with orientation/onboarding and transition to practice programs to promote employee retention within assigned site(s)
• Collaborates with recruitment, human resources, and leadership within assigned sites in verifying participants meet orientation requirements and achieve desired outcomes
• Meets regularly with key stakeholders within assigned site(s)
• Professional Role Development
• Participates and provides lifelong learning to promote continuing personal development
• Provides expertise in identifying professional development needs of individual staff
• Assists with identifying areas for individual growth in current role and grooming for advancement.
• Directs staff to career advancement resources
