Job summary

LivinRite Home Health in Fredericksburg, VA is seeking a licensed Registered Nurse to provide skilled nursing care in patients’ homes. Qualifications

  • Licensed RN in Virginia
  • Two years’ experience in home health, acute care or rehab preferred
  • Current BLS CPR certification
  • Ability to direct care for adult and geriatric populations
  • Strong written and verbal communication; confident telephone manner
  • Computer proficiency and basic office skills
  • Valid driver’s license and car insurance

Responsibilities

  • Coordinate care with Administrator, Director of Nursing and interdisciplinary team
  • Assess patient needs and support plan of care
  • Demonstrate clinical judgment to ensure safe, high-quality nursing care
  • Supervise home health aides and other staff per policy
  • Document accurately according to agency procedures
  • Communicate patient status with physicians and care team
  • Contribute to patient care conferences and multidisciplinary teamwork
  • Maintain adherence to agency policies and budget/resource stewardship

Qualifications:
  • 1. Graduate of an accepted Registered Nursing Program and licensed to practice nursing in the state of Virginia
  • 2. Minimum of two (2) years experience in home health, acute care or rehabilitation setting, preferred.
  • 3. Current BLS CPR certification.
  • 4. Must be able to direct care appropriately to support the specific populations (adult & geriatric) provided in nursing services.
  • 5. Experience in office procedures:
    • a. Ability to work independently with minimal supervision.
    • b. Ability to resolve problems and set priorities.
    • c. Excellent written and verbal skills to effectively communicate with confidence, clarity and attention to detail.
    • d. Pleasant telephone demeanor with exceptional customer service skills.
    • e. Basic typing and computer skills.
    • f. Ability to work cohesively with others and multi-task in a busy office setting.
  • 8. Current and active drivers license and car insurance.
Job Knowledge:
  • 1. Possesses a working knowledge and understanding of the functional organizational make-up of a Home Health/Hospice Agency.
  • 2. Working knowledge of State, Federal and ACHC guidelines/regulations (will train on the job.)
  • 3. Working knowledge of general computer programs (ie: Word, Excel, electronic medical record charting.)
  • 4. Working knowledge of general medical terminology.
  • 5. Strong background in Home Health, Hospice, Medical/surgical or Rehabilitation setting preferred.
  • 6. Chronic care management and wound care knowledge, skills and experience are considered a benefit for this position.
Responsibilities:
  1. Works closely with the Administrator and Director of Nursing in verifying and establishing compliance for Home Health Care and Hospice Services.
  2. Act as a comprehensive care coordinator for patients assigned to his/her care, assessing the patient s needs accordingly and facilitates communication amongst other clinicians providing care and the clinical supervisor:
    1. Demonstrate appropriate judgment skills to be able to make independent clinical decisions in patient care matters.
    2. Promote the level of clinical expertise required of staff to provide safe, exceptional and effective nursing care.
    3. Follow an appropriate plan of care which addresses the patient s functional problems.
    4. Identify appropriate short and long-term goals for each patient.
    5. Reviews results of laboratory tests and other diagnostic procedures and reports to the physician appropriately.
    6. Supervise Certified Home Health Aides (CNAs), Medical Technicians and Medical Assistants according to Federal, State, and Agency policies.
    7. Perform documentation according to the Agency s policies and procedures.
    8. Maintain regular communication with members of the interdisciplinary team regarding progress of the patient, changes in status, and discharge from services.
    9. Maintain communication with the physician regarding status of the patient including: initiation of treatment plan, changes in status, and discharge from care.
    10. Participate in patient care conferences as needed.
    11. Supervises and oversees LPN, CNA's, HHA's, etc.
  3. Keeps the Administrator and Director of Nursing apprised of updates and conflicts on a daily basis or as needed.
  4. Acts as a liaison among Agency personnel, family members, and physicians when necessary.
  5. Maintains a professional business relationship with the medical community and other health care entities.
  6. Acts as a good steward of resources, implementing innovative process improving techniques.
  7. Adheres to agency policies and procedures.
  8. Performs other duties as assigned. Must be adaptable and able to think outside the box, forward thinking, and willing to jump in and help!
