Job summary

LivinRite Home Health is seeking a full-time Licensed Practical Nurse to join our team in Virginia. The LPN will work under RN supervision to provide comprehensive home health services, including assessment, planning, and ongoing patient support from admission to discharge.

You will collaborate with patients, families, and other health professionals to ensure high-quality care, continuity of care, and cost-effective outcomes.

Qualifications

• Graduate of an accepted Licensed Practical Nursing Program and licensed to practice nursing issued by the state of Virginia
• Minimum of two (2) years experience in home health, acute care or rehabilitation setting, preferred.
• Current BLS CPR certification.
• Must be able to direct care appropriately to support the specific populations (adult & geriatric) provided in nursing services.
• Experience in office procedures: a. Ability to work independently with minimal supervision. b. Ability to resolve problems and set priorities. c. Excellent written and verbal skills to effectively communicate with confidence, clarity and attention to detail. d. Pleasant telephone demeanor with exceptional customer service skills. e. Basic typing and computer skills. f. Ability to work cohesively with others and multi-task in a busy office setting.
• 8. Current and active drivers license and car insurance.

Responsibilities

• The LPN is responsible for providing total health care services for patients after admission to discharge. They provide ongoing support and expertise through comprehensive assessment, planning, implementation and overall evaluation of individual patient needs.
• The LPN acts as a liaison between patients, family members and other health care personnel to ensure efficient and appropriate home health care services.
• Collaborate with RNs and care teams to promote continuity of care and cost effectiveness.
