Job summary

LivinRite Home Health in Virginia seeks a dedicated Licensed Practical Nurse to join our home health team. You will provide patient-centered care under RN supervision, supporting admissions through discharge and coordinating with families.

Qualifications

• Graduate of an accepted Licensed Practical Nursing Program and licensed to practice nursing issued by the state of Virginia.
• Minimum of two (2) years experience in home health, acute care or rehabilitation setting, preferred.
• Current BLS CPR certification.
• Must direct care appropriately to support adult and geriatric populations.
• Experience in office procedures: independent work, problem-solving, strong communication, customer service, typing and multitasking.

Responsibilities

• Provide total health care services for patients from admission to discharge.
• Conduct ongoing assessment, planning, implementation and evaluation of patient needs.
• Act as liaison between patients, family members and other health care personnel to ensure effective home health care services.

LivinRite is seeking an innovative and purpose driven individual to join our team as a full-time Licensed Practical Nurse, who will work collaboratively under the direction and supervision of Registered Nurse.

The LPN is responsible for providing total health care services for patients after admission to discharge. He/she provides ongoing support and expertise through comprehensive assessment, planning, implementation and overall evaluation of individual patient needs. The overall goal of this position is to enhance the quality of patient management and satisfaction, to promote continuity of care and cost effectiveness. The LPN acts as a liaison between patients, family members and other health care personnel to ensure efficient and appropriate home health care services.
