Job summary

Bristol Hospice is seeking a compassionate Registered Nurse to perform comprehensive patient and family assessments, develop and adjust care plans, and deliver skilled nursing care in a hospice setting. The role emphasizes collaboration with the interdisciplinary team and ongoing evaluation of patient needs.

Candidates should have 1+ year of acute care experience, a current state nursing license, and a willingness to travel across the Bristol Hospice service area.

Qualifications

• Must have at least 1 year of acute care experience in an institutional setting.
• One year of experience in home care preferred.
• Must have a current nursing license in the state.
• Understand the hospice philosophy and needs of the terminally ill.
• Must be willing to travel throughout the Bristol Hospice service area.

Responsibilities

• Complete an initial, comprehensive assessment of patient and family to determine hospice needs.
• Provide a complete physical assessment and history of current and previous illnesses.
• Deliver nursing care using all elements of the nursing process and revise plans as needed.
• Record pain and symptom management and communicate changes to the physician.
• Collaborate with the interdisciplinary team to plan and implement care.
