Job summary

Bristol Hospice is seeking a compassionate Registered Nurse to join our team and provide family-centered hospice care. You will perform comprehensive assessments, develop and revise care plans, and work closely with physicians and a broad interdisciplinary team to meet patient needs.

The role requires at least one year of acute care experience, current RN licensure, and a willingness to travel within our service area.

Qualifications

• Minimum 1 year of acute care experience in an institutional setting.
• Current licensure as a Registered Nurse in the state.
• Willingness and ability to travel within Bristol Hospice Service Area.

Responsibilities

• Complete an initial, comprehensive assessment of patient and family to determine hospice needs.
• Provide a complete physical assessment and history of current and previous illness(es).
• Deliver professional nursing care using the nursing process and establish a care plan.
• Record pain and symptom management and outcomes.
• Communicate with the physician regarding changes in patient status and obtain orders.
• Teach patient and family self-care techniques as appropriate.
• Work with the interdisciplinary team to coordinate care.
• Re-evaluate and revise the plan of care as patient needs change.
• Perform other duties as assigned.
