Job summary

Haven Hospice in Jacksonville, FL is seeking an On-Call Registered Nurse to provide compassionate 1:1 care during evenings, nights and weekends. You will respond to urgent symptoms, administer medications, and support families in a high-quality hospice setting.

This role requires state licensure, CPR certification, a valid driver’s license, and prior hospice experience. You will document care promptly and collaborate with physicians to adjust plans as needed.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with insured vehicle and CPR certification where required.

Responsibilities

• Collaborate with Remote Triage On Call Team to deliver hands-on care to hospice patients and offer supportive services to their families during after-hours, on an on-call basis.
• Deliver hands-on care to hospice patients and offer supportive services to their families during after-hours, on an on-call basis.
• Assess and manage urgent symptoms quickly and compassionately.
• Administer medications or adjust care per hospice protocols or provider orders.
• Communicate with the physician regarding changes in condition and obtain orders as necessary.
• Offer guidance and support to the patient and family in understanding the current condition and anticipated steps.
• Pronounce death when required during on-call hours and provide family support and arrange funeral home notification.
• Complete timely documentation and communication.

As a On-Call RN You will:

• (CHOOSE ONE: If office HAS a Remote Triage team) Collaborate with Remote Triage On Call Team to deliver hands-on care to hospice patients and offer supportive services to their families during after-hours, on an on-call basis

• (CHOOSE ONE: If office DOES NOT have Remote Triage) Deliver hands-on care to hospice patients and offer supportive services to their families during after-hours, on an on-call basis

• Assess and manage urgent symptoms quickly and compassionately

• Administer medication or adjust care based on hospice protocols or provider orders

• Communicate with the physician regarding changes in condition and obtain orders as necessary

• Offer guidance and support to the patient and family in understanding the current condition and anticipating the next steps

• If a death occurs during on-call hours, the RN will go to the home or facility to pronounce death, provide support to the family, contact the funeral home, and notify the hospice team

• Complete timely documentation and communication
