Job summary

Haven Hospice in Jacksonville, FL is seeking an On-Call Registered Nurse to provide compassionate, hands-on care during after-hours and weekends. You will manage urgent symptoms, administer medications, and coordinate with physicians to adjust care as needed.

This PRN role emphasizes 1:1 patient support in a home-based hospice setting, with emphasis on documentation, family communication, and maintaining standards of care. A valid nursing license and CPR certification are required.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• 1 year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
• Possess and maintain current CPR certification in states where required.

Responsibilities

• Assess and manage urgent symptoms quickly and compassionately.
• Administer medication or adjust care based on hospice protocols or provider orders.
• Communicate with the physician regarding changes in condition and obtain orders as necessary.
• Offer guidance and support to the patient and family in understanding the current condition and anticipating the next steps.
• If a death occurs during on-call hours, the RN will go to the home or facility to pronounce death, provide support to the family, contact the funeral home, and notify the hospice team.
• Complete timely documentation and communication.

As a On-Call RN You will:

• (CHOOSE ONE: If office HAS a Remote Triage team) Collaborate with Remote Triage On Call Team to deliver hands-on care to hospice patients and offer supportive services to their families during after-hours, on an on-call basis
• (CHOOSE ONE: If office DOES NOT have Remote Triage) Deliver hands-on care to hospice patients and offer supportive services to their families during after-hours, on an on-call basis
