Job summary

Haven Hospice in Gainesville, FL is seeking a dedicated RN Case Manager to join our team. You will provide 1:1 patient care, create and manage patient-centered hospice plans, and collaborate with an interdisciplinary team to deliver compassionate care.

The role requires a nursing license for Florida, CPR certification, and experience in hospice care. We offer a supportive environment with meaningful work and opportunities for advancement.

Qualifications

• Graduate from an accredited nursing program with current licensure to practice in the state of operation.
• One year nursing experience required; 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with insured vehicle.
• Possess and maintain current CPR certification.

Responsibilities

• Assess/ monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with the patient's wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient’s family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.

Qualifications
