Job summary

CompassionCare Hospice in Carson City, NV is seeking a compassionate Hospice Admissions RN to join our team. You will visit patients in homes, hospitals, and facilities to assess needs, explain services, obtain consents, and coordinate start of care with an interdisciplinary team.

The ideal candidate has a nursing degree and active state license, at least one year of hospice experience, and a valid driver's license.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year of hospice nursing experience required, 3+ years preferred or similar setting will be considered.
• Must possess and maintain a valid driver’s license and automobile insurance.
• CPR certification as required by state guidance.

Responsibilities

• Visit patients in homes, hospitals, or nursing facilities referred for hospice.
• Explain hospice services, discuss goals, and obtain necessary consents and documents.
• Obtain medical history, current medications, and symptoms for assessment.
• Perform comprehensive clinical assessments to determine hospice eligibility.
• Coordinate start of services (nursing, aides, equipment, medications).
• Collaborate with interdisciplinary team to plan patient care.
• Maintain accurate and timely documentation.

As a Hospice Admissions RN You will:

• Visit patients (in homes, hospitals, nursing facilities, etc.) who are referred for hospice care
• Explain hospice services, discuss goals and care with the patient and families, answer any questions, and obtain necessary consents and legal documents
• Obtain medical history, current medications, and symptoms
• Perform comprehensive clinical assessments to determine eligibility for hospice based on Medicare/insurance guidelines
• Coordinate the start of services (nursing, aides, equipment, medications.)
• Collaborate with interdisciplinary team
• Maintain accurate and timely documentation

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation
• One year of hospice nursing experience required, 3+ years preferred or similar setting such as Long-Term Care, Intensive care, or Emergency room will be considered in place of hospice nursing experience
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
• Possess and maintain current CPR certification in states where required.
