Job summary

CompassionCare Hospice in Las Vegas, NV is seeking an experienced RN Case Manager to deliver compassionate hospice care at the patient’s home or designated setting.

You will assess needs, create care plans, administer treatments, and support families, while collaborating with an interdisciplinary team and maintaining precise documentation. A valid NV RN license and CPR certification are required.

Qualifications

• Graduate from an accredited nursing program with RN licensure in NV.
• 1+ year of nursing experience; hospice experience preferred.
• Commitment to clinical excellence and thorough documentation.

Responsibilities

• Assess and monitor physical, emotional, and psychosocial needs of patients.
• Develop hospice care plans aligned with patient wishes and goals.
• Provide direct nursing care: medications, treatments, and interventions.
• Manage pain and symptoms; educate family and caregivers.
• Collaborate with interdisciplinary team and maintain timely documentation.
• Maintain on-call rotation as required by the local branch.

As a RN Case Manager You will

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation
• One year nursing experience required , 3+ years preferred
• Experience in hospice or a similar setting preferred
• Knowledge of the hospice philosophy of care
• Commitment to clinical and documentation excellence
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
• Possess and maintain current CPR certification in states where required.
