Responsibilities

An RN in this position would be responsible for providing skilled nursing care to homecare patients, the team as a whole covers sections of Essex County, NJ such as; Orange and Newark as well as some of the surrounding towns.

• Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationships with co-workers.
• Assesses and evaluates the needs of patients with multiple disease processes and comorbidities and develops an appropriate plan of care.
• Documents all changes in patient’s condition and updates care plans and outcomes.
• Identifies teaching needs and plans appropriate strategies for individuals and families.
• Monitors quality and appropriateness of services provided.
• Establishes and maintains professional working relationships with health care providers and other members of the disciplinary teams in providing patient and family care.
• Adheres to agency infection control and safety policies.
• Adheres to the organization’s policy in regards to absenteeism and appearance.
• Omission of specific duties does not exclude them from this position if the work is similar, related or a logical assignment for this position.

Qualifications

• RN with Bachelor’s degree in Nursing preferred.
• 1+ year of supervised clinical experience as a registered nurse.
• Excellent verbal and written communication skills.
• Strong computer skills.
• Excellent organizational and interpersonal skills.
• Car and valid driver’s license.

Working Conditions/Physical Demand

Working indoors and outdoors with clinical equipment and use of phone and computer; Job characteristics: Client and Customer contact.
