Job summary

Phoenix Home Care and Hospice is seeking compassionate Registered Nurses to join our home health team in Lee's Summit, MO. This role focuses on safe, patient-centered care with a collaborative interdisciplinary team and real-time charting to end workdays on time.

You will manage a personal caseload, perform routine and complex visits, educate patients and families on conditions like CHF, COPD, and diabetes, and develop plans of care with physicians.

Qualifications

• Active Missouri or Compact RN license is required.
• Home health experience strongly preferred.
• Strong clinical skills in physical assessment and diagnostic interpretation.

Responsibilities

• Manage your own caseload including supervision of LPN visits — you run your schedule.
• Varied weekly visits including routine care, evaluations, and starts of care.
• Educate patients on health conditions including CHF, COPD, Diabetes, Wound Care, and Medication Management.
• Collaborate with physicians to obtain orders and write diagnosis-driven plans of care.
• Work alongside a strong therapy team including PT, OT, and Speech.

Requirements
